I think the reason there are 4 settings in the demo but you are seeing only 3 is because we just released an update 2.2.0 for the plugin. You must have purchased and downloaded the plugin just before the update was released. You can get the latest version by downloading it from Envato.

The bulk order form will only show products that can be purchased/added to cart. So a product must have a price set up, and it must be in stock.
I see you are using the "Hide prices for guests" feature, as I see your products show "Login to view prices".
In this scenario, these products cannot be purchased or added to cart (Because they don't have a price), so they are not shown in the bulk order form either for guests.

We have just purchased your plugin and we are trying to configure it but not sure if I'm missing something.
I set all the visibility in the categories to all users but when using the bulk order form non of the products are found.
I viewed your demo environment and I see under the bulk order form you have 4 settings and our page only has 3?
