Can you let me know one thing I have created the extra fields throgh this plugin under the register form I want to show these extra billing fields on the top at the checkout page. See my screenshot below

I have one more issue on this form I have not show the "User Type" form fields on the checkout page and also same as under the checkout page popup. See my screenshot below this field is missing on checkout page and popup.

I have one more issue on the checkout time I have not see the shiiping address option on the checkout page after the billing address.And also when I have order the product at the end it give me error of first name and last name.But I have also fillup these fields. Before it was working fine .I have tested orders were done successfully.See screenshot
