(1) Regarding the visual style of the form:
Unfortunately there is no built-in editor for the design, so the only way to change the style would be with custom CSS.
I'd be happy to try and write the correct CSS to make various changes, but I'd need you to let me know which changes you'd like to make specifically.
(2) Regarding showing bank information / order number, this part does not show, because this is not an actual order, it is a quote request.
Since it is a quote request, it does not have an order number or order total.
Can you give me more information about what exactly you are hoping to achieve? How would this flow work for your business / customers ideally?
I can then advise on what the best approach for this would be using our plugin and features.
