I think generally the best option would be a plugin for free gifts, such as the plugin you mentioned at 1.
That's because we can just set B2BKing to ignore the free product in terms of pricing, so that there are no conflicts. B2BKing already has some integrations for this.
I would suggest to try it with the plugin at 1. and if you have any issues with it just let me know here, as I believe that would be something we could easily resolve.
I installed the free version of Free Gifts for WooCommerce on a staging site and indeed, it works.
I noticed in the plugin settings that there is an option to add certain conditions, in the PRO version of the plugin, depending on user role, user capability, user meta or if the client is logged in or not.
We would like these rules set with Free Gifts for WooCommerce to apply only to certain customer groups created with B2BKing.

Do you think such a thing can be achieved?
I believe you could achieve that, by syncing the groups with WP roles. We have a guide for this here: https://woocommerce-b2b-plugin.com/docs/how-to-use-wp-roles-with-b2bking/
By doing that, each group will also have a corresponding WP role, and you could then create rules based on those roles.
I'd advise to first read the above article as it explains how this works in more detail.
